Shift in Focus from Centralized Laboratory Testing to Point-of-care (POC) Testing to Propel Market Growth

In the past, testing was conducted almost exclusively in centralized laboratories. Centralized laboratory testing is very important for the optimal and consistent functioning of diagnostic processes. In recent years there is a continuous shift towards point-of-care testing services or decentralized clinical laboratory testing from centralized laboratories primarily due to the cost reduction, convenience to patients and clinicians, reduced waiting time for results, improved quality, and proliferation of point-of-care (POC) testing. POC tests can improve patient management in cases of infectious diseases. The decentralized setup also helps physicians monitor patients being treated for infectious diseases and improve clinical workflows.

The key players are developing various tests and instruments for rapid disease diagnosis. For instance, in October 2020, Siemens Healthineers AG (Germany) launched the CLINITEST Rapid COVID-19 Antigen Test to identify individuals infected with the coronavirus in 15 minutes to avoid the spread of COVID-19.

Regulatory Processes for High/Moderate-complexity Tests Likely to Hinder Market Growth

Regulatory approval processes for diagnostic devices and tests are tedious. Regulatory approval includes a review of the clinical data, calibration, manufacturing methods, quality control, risk assessment, and other factors to ensure the devices’ reproducibility and safety. Despite this, the primary regulatory hurdle for molecular diagnostic devices is the complexity. However, many institutions worldwide refuse to perform these tests due to their high complexity in POC settings.

Under the U.S. Clinical Laboratory Improvement Amendments of 1988 (CLIA), to use molecular diagnostic tests at POC settings, categorization into low-complexity tests or CLIA waived, i.e., tests that have a low risk of incorrect results is mandatory. As these tests can be used in home-care settings, they should be highly resistant to contamination and yield readily interpretable results in less time, even if used by patients directly without in-depth training on that particular assay.

Apart from the FDA, other state and accreditation agencies like the Joint Commission on Accreditation of Healthcare Organizations have additional requirements and standards for developing point-of-care molecular tests. JCI also inspects the waived status of tests. For high or moderate-complexity tests, it is mandatory to have quality checks at least at the manufacturers’ frequency. All results must be documented. With QC failures, the documentation must include what was done to rectify the problem. On the other hand, there is no need for proficiency testing for waived tests. Moreover, Dutch regulations follow quality and other technical parameters before approval for high or moderate-complexity tests, and it is mandatory that the implementation of POCT and general quality management of POC testing should be organized in collaboration with a laboratory specialist.

Thus, the strict documentation and quality auditing by regulatory bodies for high or moderate-complexity tests hinder their approval process, thereby helping develop the in-vitro diagnostics market.

In recent years there is a continuous shift towards point-of-care testing services or decentralized clinical laboratory testing from centralized laboratories primarily due to the cost reduction, convenience to patients and clinicians, reduced waiting time for results, improved quality, and proliferation of point-of-care (POC) testing.
