Segments covered: treatment type, molecule type, disease stage, etiology, route of administration, distribution channel, end user, payer.

The growth of this market is mainly driven by the rising global prevalence and mortality of chronic kidney disease, growing payer pressure to slow progression to kidney failure, expanding treatment options in glomerular and rare kidney disease, increasing label expansion across established kidney-protective agents, and strengthening guideline support for kidney-protective pharmacotherapy. However, loss of exclusivity across the largest kidney-protective franchises, volume-based procurement and price erosion in China, maximum fair price implementation under the Medicare Drug Price Negotiation Program, and low diagnosis and testing rates restrain the growth of this market.

Furthermore, closing the treatment gap among guideline-eligible patients, the growing branded opportunity in non-diabetic chronic kidney disease, expanding quality measurement of kidney health evaluation, and emerging cell and nucleic acid therapies are expected to offer growth opportunities for the stakeholders in this market. However, reimbursement classification and bundling of oral renal drugs, generic and biosimilar erosion across renal anaemia and mineral metabolism, high attrition in chronic kidney disease drug development, limited availability and standardisation of cystatin C testing, and out-of-pocket burden in emerging markets remain major challenges impacting the growth of this market. Additionally, the growing acceptance of estimated glomerular filtration rate slope as a regulatory endpoint, rising competitive density in immunoglobulin A nephropathy, increasing adoption of race-free and cystatin C-based estimation, rising deployment of home and point-of-care albuminuria testing, and growing portfolio realignment across renal specialty companies are prominent trends in this market.

The Chronic Kidney Disease Market comprises the pharmacological treatment of chronic kidney disease and its metabolic complications, together with the tests used to detect, stage and monitor the condition. The market includes kidney-protective disease-modifying therapies covering sodium-glucose co-transporter-2 inhibitors, non-steroidal mineralocorticoid receptor antagonists, glucagon-like peptide-1 receptor agonists with kidney outcome claims, renin-angiotensin system inhibitors, endothelin receptor antagonists, complement pathway inhibitors, targeted glomerular immunomodulators and vasopressin V2 receptor antagonists; complication-management therapies covering anaemia of chronic kidney disease, chronic kidney disease-mineral and bone disorder, hyperkalaemia, metabolic acidosis and chronic kidney disease-associated pruritus; and diagnosis and monitoring covering serum creatinine and estimated glomerular filtration rate, cystatin C, urine albumin-to-creatinine ratio testing, prognostic risk-stratification assays, kidney biopsy and imaging. Dialysis equipment and consumables, dialysis service delivery, kidney transplantation and transplant immunosuppression, and therapies specific to acute kidney injury are excluded. The market ecosystem extends from active pharmaceutical ingredient, peptide and biologics manufacturers to originator and specialty pharmaceutical companies, generic and biosimilar producers, diagnostics manufacturers and clinical laboratories, specialty pharmacies and distributors, payers and health technology assessment bodies, and the nephrologists, primary care prescribers and patients who diagnose and treat the condition.

The market is experiencing strong growth as chronic kidney disease moves from a condition managed by blood pressure control alone to one with multiple classes of proven kidney-protective therapy. The GBD 2023 Chronic Kidney Disease Collaborators study, published online in The Lancet on 7 November 2025, recorded 788 million adults aged 20 and above with chronic kidney disease in 2023 against 378 million in 1990, at an age-standardised prevalence of 14.2%, with stages 1 to 3 accounting for 13.9% of adults and 1.48 million deaths attributed to the condition in 2023. The KDIGO 2024 Clinical Practice Guideline, published in Kidney International at volume 105, supplement 4S, retained only six statements unchanged from its 2012 predecessor and now recommends sodium-glucose co-transporter-2 inhibitors at grade 1A across diabetic and non-diabetic chronic kidney disease above an estimated glomerular filtration rate of 20. Increasing prescribing of these classes, expanding label coverage and the arrival of branded therapy in glomerular disease are significantly increasing demand across the market.

Payer economics are further strengthening market growth, because the downstream cost of kidney failure dwarfs the cost of preventing it. Medicare spending on chronic kidney disease excluding kidney failure reached nearly USD 77 billion among beneficiaries aged 66 and above in 2021, equal to 24.1% of Medicare spending for that age group, while kidney failure accounted for a further USD 52.3 billion. Per-person Medicare spending for beneficiaries aged 66 and above was USD 28,162 with chronic kidney disease against USD 13,604 without, and mortality was 101.8 per 1,000 person-years against 46.3. More than 808,000 people in the United States live with kidney failure, of whom 68% receive dialysis and 32% hold a transplant, and global kidney replacement therapy need is projected to rise from approximately 2.6 million people in 2010 to 5.4 million by 2030. A therapy that defers that transition carries a payer value far above its acquisition cost.

Demand is being reshaped by a wave of approvals in glomerular disease, an area that carried no targeted therapy a decade ago. Novartis received traditional United States approval for Fabhalta in primary immunoglobulin A nephropathy in July 2026 after APPLAUSE-IgAN two-year results in the New England Journal of Medicine on 29 March 2026 showed an estimated glomerular filtration rate slope of negative 3.10 against negative 6.12 millilitres per minute per 1.73 square metres, 49.3% slower decline, with composite kidney failure at 21.4% against 33.5% and a hazard ratio of 0.57. Travere received approval for Filspari in focal segmental glomerulosclerosis without nephrotic syndrome in April 2026, the first approved medicine in that indication, addressing an estimated United States population above 30,000. Otsuka, Vera Therapeutics and Vertex have brought APRIL and BAFF pathway agents to approval or review, and Apellis secured the first approval in C3 glomerulopathy and primary immune complex membranoproliferative glomerulonephritis in July 2025.

Reported market value is at the same time being compressed by genericisation and administered pricing across the largest franchises. AstraZeneca reported Farxiga product revenue of USD 3,998 million for the first half of 2026, down 11% at constant exchange rates, with second-quarter revenue down 19%, attributing the decline to United States loss of exclusivity with multiple generics launched in the quarter and to volume-based procurement in China, and reported roxadustat down 63% to USD 57 million on Chinese generic competition. Otsuka reported combined tolvaptan sales of JPY 221,910 million for the year to March 2026, down 21.1% after United States generic entry in April 2025. The Centers for Medicare & Medicaid Services set a maximum fair price of USD 178.50 per 30-day supply of Farxiga against a 2023 list price of USD 556.00 and USD 197.00 for Jardiance against USD 573.00, effective for 2026.

Another major growth driver is the scale of the untreated and undiagnosed population, which sets the headroom available from existing indications alone. The Centers for Disease Control and Prevention estimates that 14% of United States adults, approximately 37 million people, have chronic kidney disease at stages 1 to 4 and that 87% do not know they have it, with prevalence reaching 38% among adults with any diabetes and 21% among those with high blood pressure. An analysis of 192,108 patients in JAMA Network Open found albuminuria testing in only 17.5% of those with hypertension or diabetes, and an Australian primary care analysis of 114,499 adults with type 2 diabetes published in Diabetes, Obesity and Metabolism found sodium-glucose co-transporter-2 inhibitors dispensed to 14.4% of those with chronic kidney disease and 17.8% of the trial-eligible subgroup. Each additional patient identified and treated represents a long-duration therapy at guideline-recommended intensity.

At the same time, the growing use of race-free and cystatin C-based estimation, home albuminuria testing and payment-linked quality measurement is reshaping the chronic kidney disease ecosystem and creating new opportunities for diagnostics suppliers. Adoption of the 2021 CKD-EPI creatinine equation among United States laboratories reached 65.8% by March 2023 from 30.3% a year earlier, while implementation of the combined creatinine and cystatin C equation stood at only 7.3%, against KDIGO grade 1B and 1C recommendations for its use. Healthy.io received clearance for a smartphone-based home urine albumin-to-creatinine ratio test on 12 July 2022 and a validation study reported sensitivity of 96.4% and specificity of 84.2%. The Kidney Health Evaluation measure, requiring both tests annually in diabetes, has been reported within Medicare Advantage and Part D Star Ratings since 2024 across plans covering more than 200 million people.

Market dynamics

23 factors across 5 forces
01

Rising Global Prevalence and Mortality of Chronic Kidney Disease

The rising global prevalence and mortality of chronic kidney disease is a major factor driving this market. The GBD 2023 Chronic Kidney Disease Collaborators study, published online in The Lancet on 7 November 2025 and covering 204 countries, recorded 788 million adults aged 20 and above living with chronic kidney disease in 2023, more than double the 378 million recorded in 1990, at an age-standardised prevalence of 14.2%. Stages 1 to 3 account for 13.9% of adults, which places most of the prevalent population in the earlier stages where pharmacological intervention can alter the trajectory. The study attributed 1.48 million deaths to chronic kidney disease in 2023, ranking it the ninth leading cause of death globally and twelfth by disability-adjusted life years at a rate of 769 per 100,000 adults, and found that impaired kidney function accounted for 11.5% of global cardiovascular deaths. Regional burden is highest across North Africa and the Middle East at 18% of adults, and high fasting plasma glucose, high body mass index and high systolic blood pressure are the leading attributable risk factors. These trends are expanding the treated population across every therapeutic class in this market.

02

Growing Payer Pressure to Slow Progression to Kidney Failure

Growing payer pressure to slow progression to kidney failure is significantly supporting demand for kidney-protective therapy. Federal costs for people with chronic kidney disease in the United States exceed USD 85.4 billion a year, equal to 23.5% of Medicare fee-for-service spending, while kidney failure alone accounted for more than 6% of Medicare spending in 2020 at more than USD 50 billion. The concentration is equally sharp elsewhere, with patients in kidney failure representing approximately 2% of those with chronic kidney disease stages 3 to 5 in the United Kingdom yet receiving half of the National Health Service chronic kidney disease budget, and with 2% to 4% of health budgets across other high- and middle-income countries directed to the 0.1% to 0.2% of the population in kidney failure. Kidney replacement therapy need is projected to rise from approximately 2.6 million people in 2010 to 5.4 million by 2030, and fewer than half of those requiring it can currently access it. Because a therapy that defers dialysis displaces a far larger downstream cost, payers are increasingly willing to fund early-stage pharmacotherapy, supported by a 2021 meta-analysis finding that sodium-glucose co-transporter-2 inhibitors reduce the risk of chronic kidney disease progression by 37%.

03

Expanding Treatment Options in Glomerular and Rare Kidney Disease

The rapid expansion of approved treatment options in glomerular and rare kidney disease is a major factor driving this market, as indications that carried no approved therapy a few years ago now support competing branded products. In immunoglobulin A nephropathy, Novartis received traditional United States approval for Fabhalta in primary disease in July 2026, the first complement inhibitor shown to slow kidney function decline in this population, after APPLAUSE-IgAN two-year results published in the New England Journal of Medicine on 29 March 2026 reported an estimated glomerular filtration rate slope of negative 3.10 against negative 6.12 millilitres per minute per 1.73 square metres per year, a difference of 3.02 equal to 49.3% slower decline, with the composite kidney failure endpoint at 21.4% against 33.5% and a hazard ratio of 0.57. Otsuka received accelerated approval for sibeprenlimab in November 2025 as the first anti-APRIL antibody in the indication, Vera Therapeutics received accelerated approval for atacicept on 7 July 2026, and Vertex had a biologics licence application for povetacicept accepted on 1 June 2026 with a target action date of 30 November 2026. These launches are establishing a branded segment in a disease previously managed with supportive care.

04

Increasing Label Expansion Across Established Kidney-Protective Agents

Increasing label expansion across established kidney-protective agents is significantly supporting demand by widening the treatable population. Bayer secured a third indication for Kerendia in September 2026, covering reduction of urine albumin-to-creatinine ratio in adults with chronic kidney disease associated with type 1 diabetes, alongside its existing indications in chronic kidney disease associated with type 2 diabetes and in heart failure with left ventricular ejection fraction of 40% or above. The company also reported FIND-CKD on 5 June 2026, a Phase III study in 1,584 adults with non-diabetic chronic kidney disease on maximally tolerated renin-angiotensin blockade, in which the estimated glomerular filtration rate slope was negative 3.3 against negative 4.0 millilitres per minute per 1.73 square metres per year, a difference of 0.7 with a confidence interval of 0.3 to 1.1 and a p-value below 0.001, with a key secondary composite hazard ratio of 0.77. Bayer notes that more than 37 million adults in the United States have chronic kidney disease and that an estimated 50% to 70% of them do not have diabetes, which defines the scale of the population these expansions address.

05

Strengthening Guideline Support for Kidney-Protective Pharmacotherapy

Strengthening guideline support for kidney-protective pharmacotherapy is significantly increasing the eligible prescribing population. The KDIGO 2024 Clinical Practice Guideline for the Evaluation and Management of Chronic Kidney Disease, published in Kidney International at volume 105, supplement 4S, pages S117 to S314, retained only six statements unchanged from the 2012 edition. It recommends sodium-glucose co-transporter-2 inhibitors at grade 1A for adults with type 2 diabetes and chronic kidney disease at an estimated glomerular filtration rate of 20 or above, and at the same grade for adults with chronic kidney disease at an estimated glomerular filtration rate of 20 or above with an albumin-to-creatinine ratio of 200 milligrams per gram or above, or with heart failure irrespective of albuminuria, with a grade 2B suggestion extending to an estimated glomerular filtration rate of 20 to 45 at a ratio below 200. Non-steroidal mineralocorticoid receptor antagonists carry a grade 2A suggestion in type 2 diabetes above an estimated glomerular filtration rate of 25 with albuminuria above 30 milligrams per gram despite maximally tolerated renin-angiotensin blockade, and long-acting glucagon-like peptide-1 receptor agonists carry grade 1B. The guideline also lowered the metabolic acidosis treatment threshold to a serum bicarbonate below 18 millimoles per litre.

01

By Treatment Type

  • The disease-modifying and kidney-protective therapies segment is expected to account for the largest share of the global Chronic Kidney Disease Market.
  • The large share of this segment is mainly due to the revenue scale of sodium-glucose co-transporter-2 inhibitors, non-steroidal mineralocorticoid receptor antagonists and glucagon-like peptide-1 receptor agonists, with AstraZeneca reporting Farxiga first-half 2026 revenue of USD 3,998 million and Bayer reporting Kerendia first-half sales of EUR 603 million, up 75.3%.
  • However, the diagnosis and monitoring segment is projected to register the highest CAGR during the forecast period.
  • The rapid growth of this segment is attributed to the Kidney Health Evaluation measure requiring both estimated glomerular filtration rate and urine albumin-to-creatinine ratio testing annually in diabetes, now reported within Medicare Advantage and Part D Star Ratings across plans covering more than 200 million people, and to albuminuria testing standing at only 17.5% among at-risk adults.

02

By Molecule Type

  • The small molecules segment is expected to account for the largest share of the market.
  • The large share of this segment is mainly due to the oral administration of the sodium-glucose co-transporter-2 inhibitors, non-steroidal mineralocorticoid receptor antagonists, endothelin receptor antagonists, complement factor B inhibitors, phosphate absorption inhibitors and hypoxia-inducible factor prolyl hydroxylase inhibitors that dominate prescribing.
  • However, the biologics segment is projected to register the highest CAGR during the forecast period.
  • The rapid growth of this segment is attributed to the arrival of APRIL and BAFF pathway agents in immunoglobulin A nephropathy, with Otsuka receiving accelerated approval for sibeprenlimab in November 2025, Vera Therapeutics receiving accelerated approval for atacicept on 7 July 2026 at a dose of 150 milligrams administered subcutaneously each week by autoinjector, and Vertex holding a target action date of 30 November 2026 for povetacicept following a Week 36 interim showing a placebo-adjusted urine protein-to-creatinine ratio reduction of 49.8%.
03

By Disease Stage

  • The stage 3a and stage 3b segments together are expected to account for the largest share of the market.
  • The large share of these stages is mainly due to the concentration of the prevalent population in earlier disease, with the GBD 2023 study placing stages 1 to 3 at 13.9% of adults against an overall age-standardised prevalence of 14.2%, and to the KDIGO 2024 recommendation of sodium-glucose co-transporter-2 inhibitors at grade 1A above an estimated glomerular filtration rate of 20.
  • However, the stage 1 and stage 2 segment is projected to register the highest CAGR during the forecast period.
  • The rapid growth of this segment is attributed to earlier detection through albuminuria screening, since the KDIGO risk classification assigns high and very high risk to albuminuria category A3 even at a preserved glomerular filtration rate.
04

By Route of Administration

  • Market Analysis by Route of Administration
  • The oral segment is expected to account for the largest share of the market.
  • The large share of this segment is mainly due to the oral dosing of every major kidney-protective class and of the phosphate binders, potassium binders and hypoxia-inducible factor prolyl hydroxylase inhibitors used in complication management, which supports long-duration community prescribing.
  • However, the subcutaneous segment is projected to register the highest CAGR during the forecast period.
  • The rapid growth of this segment is attributed to the glucagon-like peptide-1 receptor agonists carrying a KDIGO grade 1B recommendation and to the biologic agents entering glomerular disease, with atacicept administered at 150 milligrams weekly by self-administered autoinjector and povetacicept studied at 80 milligrams every four weeks in the RAINIER trial of 605 adults.
05

By Distribution Channel

  • The retail pharmacies segment is expected to account for the largest share of the market.
  • The large share of this segment is mainly due to the chronic oral prescribing of kidney-protective and complication therapies in community settings, where the Centers for Medicare & Medicaid Services maximum fair prices of USD 178.50 for a 30-day supply of Farxiga and USD 197.00 for Jardiance now apply.
  • However, the specialty pharmacies segment is projected to register the highest CAGR during the forecast period.
  • The rapid growth of this segment is attributed to the launch of high-cost biologics and orphan-designated agents in glomerular disease that require benefit verification, patient support and distribution control, with Vera Therapeutics operating a dedicated copay programme for atacicept and Novartis distributing iptacopan for C3 glomerulopathy under a risk evaluation and mitigation strategy.
06

By End User

  • The specialty and ambulatory clinics segment is expected to account for the largest share of the market.
  • The large share of this segment is mainly due to the concentration of nephrology prescribing and of glomerular disease management in specialist outpatient settings, where biopsy-confirmed diagnosis and proteinuria monitoring are performed.
  • However, the primary care and community health settings segment is projected to register the highest CAGR during the forecast period.
  • The rapid growth of this segment is attributed to the shift of case finding and first-line kidney-protective prescribing into primary care, supported by the Kidney Health Evaluation measure in the Merit-based Incentive Payment System since 2023, and by evidence that up to half of advanced chronic kidney disease remains undetected in primary care.

Geographic analysis

01

North America

Largest share

In 2026, North America is expected to account for the largest share of the global Chronic Kidney Disease Market, at an estimated 45.0%. The regional position is supported by prevailing drug prices, broad formulary coverage of kidney-protective therapy and the concentration of launches in glomerular disease. The Centers for Disease Control and Prevention estimates that 14% of United States adults, approximately 37 million people, have chronic kidney disease at stages 1 to 4, with 87% unaware of the diagnosis, and Medicare spent nearly USD 77 billion on chronic kidney disease excluding kidney failure among beneficiaries aged 66 and above in 2021 alongside USD 52.3 billion on kidney failure. Payment policy is now a material variable, with maximum fair prices of USD 178.50 for a 30-day supply of Farxiga and USD 197.00 for Jardiance effective for 2026, semaglutide selected for 2027, and the calendar year 2026 End-Stage Renal Disease Prospective Payment System base rate set at USD 281.71. The End-Stage Renal Disease Treatment Choices model was terminated early with effect from 31 December 2025. North America

02

Europe

Europe is expected to account for a significant share of the market, at an estimated 24.0% in 2026, and carries the most structured health technology assessment environment for kidney-protective therapy. The National Institute for Health and Care Excellence published TA1075 on 2 July 2025 recommending dapagliflozin as an option added to optimised standard care for adults with an estimated glomerular filtration rate of 20 to below 45, or 45 to 90 with a urine albumin-to-creatinine ratio of 22.6 milligrams per millimole or above or type 2 diabetes. The European Commission converted the Filspari marketing authorisation from conditional to standard, announced on 29 April 2025, for adults with primary immunoglobulin A nephropathy with urine protein of 1.0 gram per day or above, a population the licence holder places at up to 250,000 people across its European, Australian and New Zealand territories. The Committee for Medicinal Products for Human Use issued a positive opinion on a Kerendia variation at its meeting of 29 January 2026. Reported prevalence includes 11.2% in the Hamburg City Health Study of 9,983 participants and approximately 7.2 million people in the United Kingdom. Europe

03

Asia-Pacific

Fastest growth

However, Asia-Pacific is projected to register the highest CAGR during the forecast period, from an estimated 22.5% share in 2026. The rapid growth of this region is attributed to the scale of the prevalent population and to widening reimbursement coverage. An Indian parliamentary standing committee report on chronic kidney disease, presented on 12 August 2026, identified the condition as the ninth leading cause of mortality in India against approximately 115 million cases recorded in 2017, and recommended a national strategy with a district-wise performance dashboard and delivery through Ayushman Arogya Mandirs, alongside the Pradhan Mantri National Dialysis Programme which covered 751 districts through 1,704 centres as at 30 June 2025. Japanese health-checkup data covering 761,565 records found age-adjusted prevalence of 71.8 per 1,000 people under 75, with chronic kidney disease diagnosis codes recorded for only 23.2% of those affected. China issued its 2025 National Reimbursement Drug List on 7 December 2025, effective 1 January 2026, covering 3,253 drugs with 114 newly added, alongside a first commercial insurance innovative drug list of 19 products. Asia-Pacific

04

Latin America

Latin America is expected to account for an estimated 4.5% share of the market in 2026, constrained principally by formulary access rather than by disease burden. Brazilian prevalence estimates include 6.7% of adults with an estimated glomerular filtration rate below 60 in the National Health Survey of 2014 to 2015 across 7,457 adults, rising to 21.4% among those over 60, and 8.9% in a multicentre study of more than 14,500 participants aged 35 to 74 across six states, with up to 44% of early-stage disease estimated to remain undiagnosed. The public health system supplies angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, statins, metformin and insulins without charge and provides calcitriol, sevelamer and erythropoietin through its high-cost programme, but sodium-glucose co-transporter-2 inhibitor access remains narrow, with dapagliflozin approved under Portaria No. 16 of 29 April 2020 only for type 2 diabetes in patients aged 65 and above with cardiovascular disease inadequately controlled on optimised metformin and sulfonylurea. Latin America

05

Middle East & Africa

The Middle East & Africa is expected to account for an estimated 4.0% share of the market in 2026 and carries the highest regional disease burden alongside the weakest financial protection. The GBD 2023 study identified North Africa and the Middle East as the region with the highest prevalence, affecting 18% of adults, well above the global age-standardised figure of 14.2%. Access is severely constrained by out-of-pocket exposure. A prospective costing analysis across six Ethiopian facilities published in BMJ Global Health recruited 433 patients in early 2023 and recorded mean annual out-of-pocket expenditure of USD 2,337, with catastrophic health expenditure at a 10% consumption threshold affecting 55.9% of households overall and 90.2% of those on haemodialysis, and 25.6% of households pushed below the poverty line. A Saudi single-centre cohort of 1,010 adults with diabetes recorded albumin-to-creatinine ratio screening in 49.3% of patients, with renin-angiotensin inhibitor use in 78.9% of those found to have albuminuria. Competitive landscape

The global Chronic Kidney Disease Market is moderately fragmented, with competition among large-cap pharmaceutical companies holding cardio-renal-metabolic franchises, nephrology-focused specialty pharmaceutical companies, rare kidney disease and glomerular disease developers, renal anaemia and mineral metabolism suppliers, generic and biosimilar manufacturers, and in vitro diagnostics and laboratory service providers. Market participants compete primarily on the strength of kidney outcome evidence, guideline recommendation grade, breadth of label across diabetic and non-diabetic disease, reimbursement listing and health technology assessment outcomes, duration of market exclusivity, specialty distribution and patient support capability, and the ability to support long-duration therapy through monitoring and adherence programmes.

Leading companies are investing in label expansion, glomerular disease pipelines and kidney outcome trials while reallocating capital away from genericised complication franchises. Bayer secured a third Kerendia indication in September 2026 covering chronic kidney disease associated with type 1 diabetes and reported FIND-CKD in non-diabetic disease across 1,584 patients on 5 June 2026. Novartis obtained traditional United States approval for Fabhalta in primary immunoglobulin A nephropathy in July 2026 and completed submissions for traditional approval of atrasentan in the United States and European Union. Consolidation has been pronounced. Biogen completed its acquisition of Apellis Pharmaceuticals on 14 May 2026 at USD 41.00 per share in cash plus a contingent value right of up to USD 4.00 per share, Asahi Kasei consolidated Calliditas at an acquisition cost of JPY 167.8 billion, Travere licensed civorebrutinib from Everest Medicines for USD 112.5 million upfront in July 2026, and FibroGen completed the sale of FibroGen China to AstraZeneca on 2 September 2025 for approximately USD 220 million. CSL recorded a pre-tax impairment of USD 4,869 million against CSL Vifor and guided its revenue down approximately 25% for the 2027 financial year.

Expert perspectives

The Chronic Kidney Disease Market is entering a period in which the evidence for treating the disease is settled and the constraint has moved to finding the patients and holding the price. The KDIGO 2024 guideline recommends sodium-glucose co-transporter-2 inhibitors at grade 1A across diabetic and non-diabetic disease above an estimated glomerular filtration rate of 20, yet an Australian primary care analysis of 114,499 adults with type 2 diabetes found the class dispensed to 14.4% of those with chronic kidney disease, and the Centers for Disease Control and Prevention estimates that 87% of affected United States adults do not know they have the condition. At the same time the agents carrying that evidence have reached the end of their exclusivity, with Farxiga down 11% at constant exchange rates in the first half of 2026 on United States genericisation and a maximum fair price 68% below its 2023 list price.

Three structural changes are expected to shape the market through 2036. First, value is migrating from the genericising cardio-renal agents toward glomerular disease, where four mechanisms now hold approval or are under review in immunoglobulin A nephropathy alone and where reported growth runs at 88% to 96% year on year against single-digit or negative growth in the established classes. Second, diagnosis is becoming a payment-linked activity rather than a clinical discretion, as the Kidney Health Evaluation measure enters Medicare Advantage and Part D Star Ratings and home albuminuria testing raises screening completion from 21.3% to 53.1% in controlled comparison. Third, the regulatory pathway has shortened, with estimated glomerular filtration rate slope accepted as a surrogate and accelerated approvals granted in sequence across immunoglobulin A nephropathy, C3 glomerulopathy and focal segmental glomerulosclerosis.

For companies planning entry or expansion, the most attractive positions over the forecast period are likely to be found in glomerular and hereditary kidney disease where orphan designation protects pricing, in non-diabetic chronic kidney disease where 50% to 70% of the affected population falls outside the diabetes label, in albuminuria case-finding and cystatin C confirmatory testing where implementation stands at 7.3% of United States laboratories, and in long-duration adherence and monitoring services. The principal risks are the concentration of current value in agents already subject to genericisation and administered pricing, the bundling of oral renal drugs into the End-Stage Renal Disease Prospective Payment System with transitional add-on payments expiring, the historic attrition rate of kidney outcome trials illustrated by the failure of VALOR-CKD at a hazard ratio of 0.99, and out-of-pocket exposure that places catastrophic cost on 55.9% of affected households in low-income settings.

Customer perspectives

Insights gathered during primary interviews with nephrologists, formulary directors, payer medical officers and laboratory directors operating in this market highlight where purchasing priorities are shifting. The following perspectives reflect recurring themes raised across these discussions.

Customer perspective
“This reflects a shift in which payment-linked quality measurement, rather than specialist capacity, increasingly determines how early patients enter kidney-protective therapy.”
Chief of Nephrology · Academic Medical Centre, United States
Customer perspective
“This indicates that payers are differentiating within the glomerular disease class on durable kidney function evidence, not on approval status alone.”
Pharmacy Director, Formulary Management · Regional Health Plan, Europe
Customer perspective
“This points to reagent cost and order-set design, and not guideline wording, as the practical constraint on cystatin C implementation.”
Laboratory Director · Integrated Delivery Network, Asia-Pacific
